Patients are referred to INTEGRIS Health Jim Thorpe Rehabilitation through their attending physicians or discharge coordinators, including social workers or case managers, primarily from acute care hospitals or other health care facilities where they are currently receiving treatment.
In some cases, patients may also directly access our services through their primary care physician.
