If you are currently on a treatment plan with an INTEGRIS Health doctor or receiving treatments at an INTEGRIS Health facility, we encourage you to contact UnitedHealthcare to initiate a Continuity of Care request. If UnitedHealthcare approves your request, you may continue to see your INTEGRIS Health doctor at an in-network benefit rate after the termination date.

Notice to UnitedHealthcare Patients

After March 9, 2026, INTEGRIS Health will be out-of-network and no longer accepting UnitedHealthcare insurance, including:

  • Commercial plans
  • Medicare (AARP) plans
  • Marketplace plans

Transition of Care / Continuity of Care Options

You may request that UnitedHealthcare grant a Transition of Care (Continuity of Care) extension. To do so, please submit a request form to UnitedHealthcare.

  • Forms are available on the UnitedHealthcare website (www.uhc.com).
  • Requests must be submitted within 30 days of the effective date of coverage change or within 30 days of the provider’s termination date.
  • A separate form is required for each medical condition.

Transition of Care may be approved in certain situations, including if:

  • You are undergoing treatment for a serious and complex condition, including an acute or chronic illness;
  • You have a degenerative or disabling condition or disease;
  • You are pregnant and undergoing treatment for pregnancy at INTEGRIS Health;
  • You are scheduled to undergo non-elective surgery; or
  • You are terminally ill.
