Join us at one of our free weight loss seminars. After completing a seminar, we recommend contacting your insurance provider regarding your bariatric benefits. If your insurance covers surgical weight loss, below are testing and documents you should start completing.

• Chest X-ray
• Labs (CMP, CBC, Iron and TIBC, TSH, Fasting Lipid Panel, A1C, Vitamin D, 25 Hydroxy, Vitamin B12/folate, Vitamin B1)
• EKG
• PFT (All Medicare patients and previous smokers)
• Primary care physician (PCP) letter (view a sample letter). Discuss your decision with your family physician. Your PCP's support is important.
• Attend an INTEGRIS Health Weight Loss Center support group
