Care Coordination participants on five or more chronic medications or not at treatment goals for asthma, diabetes, hyperlipidemia, or hypertension will be referred for a visit.
For a Medication Management Clinical Pharmacist to see your patients, the physician and the pharmacists must enter into a Collaborative Practice Agreement.
